DDA Qualified Services Provider Application and Provider Renewal

The application for a new agency or individual provider, a renewal, or adding services. Employment Services are among the DDA-certified services: Discovery, Job Development, Co-worker Employment Services, Self-Employment Development Supports, Follow Along Supports and Ongoing Job Supports. Agencies also submit a Program Service Plan, a Quality Assurance Plan and budgets (attachments 1 to 22). The form notes that payment may not arrive for up to 180 days after services begin.
